Physician Letters
Below are links to letters that you can take to your family physician to help explain what medical care you may need to prepare you for travel.
If you are traveling to a:
- Low risk for exposure area* such as United Kingdom, Australia, New Zealand, Japan, or Western Europe bring the Low Risk Letter to your physician.
- High risk for exposure area* such as Asia/India, Africa, Central America, Eastern Europe, or South America bring the High Risk Letter to your physician.
*To vaccine preventable diseases and insect carried diseases.
