Preparation for Your Appointment (Required)
Below is information you will need to locate, complete, and bring to your travel appointment:
- A completed travel vaccine planner
- Review the inforation in the Health Information for Travelers section that is on this Web site.
- Review the costs that will be associated with your travel appoinmtent. Travel appointments have costs associated with the appointment whether it is simply the consultation or also vaccines. Payment is due at the time of service. The Travel Clinic does not bill insurance.
- Copy of childhood and adult vaccination records.
- Payment: Travel Clinic does not bill insurance. Bring cash, check, or a credit card for payment. Or, if you are a MSU faculty or staff member, traveling on University business, bring the departmental account number to which this appointment will be charged.
- Arrive to your appointment at least 15 minutes early. This will allow sufficient time to check in for your appointment.
